Country Oaks at Hamilton Creek offers affordable quality assisted living care for adults who need assistance with activities of daily living but do not require the full service of a skilled nursing home. We offer two levels of care service plans and additional services based on individual needs.
Our Care Service Plans
Each plan has a different level of care. Choose the plan that works best for you.
Plan I: $300 per month
Features:
- Medication assistance
- Standby bathing assistance
- Standby dressing assistance
- Standby grooming assistance
Plan II: $540 per month
Features:
- Medication assistance
- Complete bathing assistance
- Complete grooming assistance
- Incontinence care
Additional Services
Local Transportation
- Scheduled local transportation (6 times per month)
- Scheduled local transportation more than 6 times per month will cost $20 per additional transport
Transportation to Austin, Texas, for Appointments
- Transportation to appointments to Austin or outside the local area will cost $180.00 per trip, plus an additional $30 per hour after the first 3 hours
Emergency Pendant
- One-time non-refundable fee of $300.00
- Pendant remains property of County Oaks at Hamilton Creek
