We offer a selection of room layouts, including studio, one bedroom, and two-bedroom floor plans, as well as a wide range of activities, amenities, and care services for our residents. Quality Care You Can Count on
Our caring professional staff includes attendants who are highly trained and on duty 24 hours a day, seven days a week to assist our residents. We provide restorative care, which is a lower level of therapy than physical therapy. Restorative care can help residents improve after physical therapy concludes.
We partner with the home health agency of your choice for additional licensed or skilled services. We also partner with hospice agencies to provide an array of services to our resident who may face a terminal illness, allowing them to remain in their home while getting the attention and care they deserve during this stage of life. Hospice agencies work with residents and families to provide comfort care and other services with dignity.
We offer resident services and care services for standby assistance or complete assistance for dressing, bathing, grooming, and incontinence care. Our residents enjoy a wide variety of amenities and activities to keep themselves entertained, happy, and fulfilled.
Aging in Place
Our facility's philosophy is to allow residents to "age in place" and remain at Country Oaks at Hamilton Creek, their home, for as long as they desire. This gives them the freedom to be independent for as long as they can while also having access to assistance from compassionate, friendly staff whenever necessary.
